What you will do

• Facilitate safe, timely, and effective hospital discharges.

• Oversee and manage the Extra Care Housing Step-Down Pathway, ensuring recovery- and reablement-focused interventions.

• Conduct strengths-based reviews and reassessments, ensuring support remains aligned with residents’ current needs.

• Complete annual planned reviews for residents, promoting independence and capturing desired outcomes.

• Identify opportunities to promote independence through assistive technology, community resources, and informal networks.

• Explore alternative support arrangements to achieve positive outcomes.

• Coordinate person-centred support and discharge planning with health and social care partners.

• Apply Care Act 2014 principles to assessment, support planning, and review processes.

• Manage complex situations, balancing risk, choice, and control while safeguarding individuals’ rights.

The aim is to ensure adults receive support tailored to their strengths, needs, and aspirations, promoting independence and facilitating the flow of people through health and social care services.

Requirements

The successful candidate will be a dynamic Social Worker with:

• Registration with Social Work England.

• Experience in hospital discharge, adult social care reviews, and managing complex cases.

• A thorough understanding of the Care Act 2014, Mental Capacity Act 2005, and safeguarding duties.

• Strong assessment, analytical, and decision-making capabilities.

• Experience working in multidisciplinary and partnership settings.

• A commitment to strengths-based practice and achieving positive outcomes for residents.
