What you will do:
- Assess the needs and risks of adolescents and their families to develop appropriate care plans.
- Provide ongoing support and intervention to adolescents in need, including crisis management.
- Collaborate with a range of professionals and external agencies to ensure a holistic approach to care.
- Maintain comprehensive case records and produce reports as required for reviews and legal proceedings.
- Advocate for the young people and their families, ensuring their voices are heard and respected.
