What you will do

• Social Care Reviews: Conduct comprehensive social care reviews, ensuring services remain appropriate and effective.
• Assessment: Complete Care Act assessments and contribute to the development of support plans that meet individual needs.
• Case Management: Manage a caseload of approximately 15 to 18 adults, ensuring timely interventions and high-quality outcomes.
• Partnership Working: Collaborate closely with the Floating Support Team to provide coordinated and effective support.
• Care Planning: Promote independence and wellbeing through person-centred care planning and review processes.
• Legislative Compliance: Work in accordance with the Care Act 2014, safeguarding procedures, and Adult Social Care legislation.
• Record Keeping: Maintain accurate, timely, and compliant case records, assessments, and reports.
• Outcome-Focused Practice: Support individuals to achieve positive outcomes while maximising independence and choice.

Qualifications / Requirements

• Social Work Qualification: Degree in Social Work or equivalent recognised qualification.
• Registration: Current Social Work England registration.
• Experience: Minimum of two years' post-qualification experience within Adult Social Care.
• Adult Social Care Knowledge: Strong understanding of reviews, Care Act assessments, and adult social care processes.
• Assessment Skills: Experience undertaking strengths-based assessments and person-centred support planning.
• Communication Skills: Excellent written, verbal, and interpersonal abilities.
• Organisational Skills: Ability to manage competing priorities and maintain high standards of practice.
