What you will do
• Referral Management: Screen and prioritise incoming referrals, making timely decisions based on urgency, risk, and need.
• Urgent Interventions: Support urgent cases, including hospital discharge facilitation and admission prevention initiatives.
• Assessment: Conduct comprehensive occupational therapy assessments focused on independence, mobility, and activities of daily living.
• Equipment Provision: Recommend and arrange community equipment and assistive technology to maximise independence and safety.
• Adaptations Assessment: Assess for minor and major adaptations and facilitate appropriate referrals where required.
• Multi-Agency Working: Work collaboratively with service users, carers, healthcare professionals, and partner organisations.
• Care Planning: Develop practical solutions and interventions to support individuals in achieving positive outcomes.
• Record Keeping: Maintain accurate and timely documentation in line with the Care Act 2014 and local authority requirements.
• Safeguarding: Identify and respond appropriately to safeguarding concerns, ensuring individuals receive the necessary support and protection.
Qualifications / Requirements
• Occupational Therapy Qualification: Degree in Occupational Therapy or equivalent recognised qualification.
• Registration: Current HCPC registration as an Occupational Therapist.
• Experience: Previous experience working within Adult Social Care, community services, or a duty team environment.
• Legislative Knowledge: Strong understanding of the Care Act 2014 and relevant adult social care legislation.
• Assessment Skills: Experience conducting functional assessments, equipment assessments, and adaptation recommendations.
• Communication Skills: Excellent verbal, written, and interpersonal communication abilities.
• Organisational Skills: Ability to prioritise competing demands and manage urgent referrals effectively.
• Multi-Agency Working: Experience collaborating with multidisciplinary teams and external agencies to achieve positive outcomes.
