Working on a hybrid basis with three days per week in the office across Market Drayton and Oswestry, this role is ideal for an experienced social worker looking to deliver high-quality, strengths-based practice within a supportive Adult Social Care team.

Working from 03 August 2026 until 03 November 2026, you will support adults with disabilities, older people, and their carers, helping them to maximise independence and remain connected to their communities.

What you will do:

• Care Act Assessments: Undertake comprehensive assessments and fulfil statutory duties under the Care Act 2014.

• Support Planning: Develop and review person-centred care and support plans that promote independence and wellbeing.

• Safeguarding: Contribute to Multi-Agency Protection of Vulnerable Adults procedures and safeguarding investigations.

• Case Management: Manage a varied caseload of adults with disabilities, older people, and their carers.

• Strengths-Based Practice: Apply strengths-based approaches to empower individuals and maximise independence.

• Hospital Discharge Support: Work collaboratively with hospital teams to facilitate safe, timely, and effective discharges.

• Partnership Working: Liaise with health professionals, community organisations, carers, and partner agencies to coordinate support.

• Risk Assessment: Identify and manage risks while promoting choice, control, and positive outcomes.

• Customer Engagement: Ensure service users are actively involved in assessment, planning, and decision-making processes.

• Record Keeping: Maintain accurate, timely, and compliant case records, reports, and documentation.
