About the Role:
Westminster City Council is on the lookout for an experienced and motivated Mental Health Social Worker to collaborate with our dedicated multi-disciplinary team. Your role will involve providing steadfast support to our community's residents, ensuring they achieve positive outcomes and enhancing their quality of life. You'll partner with health services, community organisations, and our internal teams, all while handling a diverse and rewarding caseload.
What you will do:
- Participate actively in the daily duty rota and manage incoming calls and referrals efficiently
- Conduct thorough Care Act strengths-based assessments
- Create and review detailed Care Act care and support plans
- Apply for funding for placements when necessary
- Collaborate with health colleagues to develop Section 117 aftercare plans
- Attend MDT ward rounds and community MDT meetings regularly
- Plan safe and timely discharges for individuals from acute or psychiatric hospital settings
- Act as a Lead Enquiry Officer for safeguarding enquiries, ensuring thorough investigations
- Carry out Carers Assessments under the Care Act to support family carers
- Undertake Mental Capacity Act assessments with diligence and care
- Engage in team meetings, sharing insights, and aiding team development

Essential Skills, Qualifications and Experience:
- Registration with Social Work England is a must
- Proven experience in a community mental health team
- In-depth knowledge of the Care Act, Mental Capacity Act, and safeguarding
- Ability to collaborate effectively with health professionals and multi-disciplinary teams
- Strong organisational skills and a knack for thriving in a fast-paced environment
