The Rise Crisis and Home Treatment Team offers vital multi-disciplinary support to children and young people under 18 who are navigating a mental health crisis. In this role, you will have the opportunity to work with individuals at significant risk to themselves or others, those being considered for mental health inpatient admission, and individuals at risk of self-harm.
What you will do:
- Conduct comprehensive mental health assessments for children and young people.
- Develop and implement crisis management plans.
- Work collaboratively with a multi-disciplinary team including doctors, nurses, and social workers.
- Provide therapeutic interventions and support to young people and their families.
- Liaise with external agencies to ensure coordinated care for service users.
