What you will do:

Undertake Care Act 2014 assessments to identify needs, eligibility, and appropriate support.

Manage a caseload of adults with learning disabilities and autism, including complex cases.

Develop and review care and support plans promoting independence and community inclusion.

Lead and respond to safeguarding enquiries under Section 42, ensuring effective risk management.
Person Specification:
-
Degree in Social Work (BA/MA or equivalent) with evidence of continued professional development.

Registered with Social Work England.

Knowledge of the Care Act 2014, Mental Capacity Act 2005, and Autism Act 2009.

Enhanced DBS clearance required.
