What you will do:
- Manage a diverse caseload, providing tailored support to individuals with learning disabilities and their carers.
- Conduct comprehensive assessments of care and support needs, ensuring compliance with the Care Act 2014.
- Collaborate with individuals to develop and implement care plans, aiming for their highest level of independence.
- Conduct financial assessments and provide guidance on service costs and welfare entitlements.
- Arrange the provision and maintenance of assistive technology and minor adaptations, ensuring these are effectively used to meet individuals' needs.
- Actively partake in adult safeguarding enquiries, contributing to the creation and implementation of safeguarding plans.
- Exercise professional judgements within the legal and policy frameworks, advocating for clients' best interests.
- Keep meticulous records and reports, essential for performance monitoring and legal proceedings.
- Engage in continuous learning and development, staying current with evolving policies and social work practices.
