Job Title: Social Worker - Initial Assessment Team, Swansea

Exciting opportunity for a Social Worker in the Initial Assessment Team in Swansea, earning an attractive £38.15 per hour on an ongoing contract. The office is located at Guildhall, Swansea but be prepared for some flexibility within the team, either supporting the CAP Assessment Team or engaging with the CAP MDT/Rapid Response.

What you will do:

  • Conduct initial assessments and engage with individuals and families to determine needs and support plans
  • Collaborate within the team to manage cases effectively, ensuring that services provided meet compliance standards
  • Communicate with other professionals, agencies, and the community to support service users and their families
  • Document and maintain accurate records of work and decisions taken in line with council and legislative requirements
  • Provide immediate response to crisis situations, ensuring the safety and well-being of service users

Requirements of the Social Worker:

  • Degree or equivalent qualification in social work
  • Current registration with Social Care Wales
  • Significant frontline experience in UK Social Services as a qualified Social Worker
