What you will do:

• Become an integral part of the hospital discharge team, working efficiently to facilitate smooth and safe transitions for patients returning home.
• Complete comprehensive assessments and reviews, including Decision Support Tool (DST) assessments, and present findings to the panel.
• Work both in the office environment on Wednesdays and in the community, where you'll need access to transport to reach clients and other professionals.
• Collaborate with a multidisciplinary team, sharing insights and strategies to enhance patient care and support.
