What you will do:

- Conduct thorough assessments in compliance with the Care Act to determine individuals' care and support needs.
- Develop and review personalised care and support plans focusing on person-centred outcomes.
- Engage in safeguarding enquiries to manage risks effectively and ensure the well-being of adults.
- Perform mental capacity assessments and make informed best interest decisions under the Mental Capacity Act.
- Manage a diverse caseload of adults with complex needs requiring expert oversight and support.
- Facilitate smooth hospital discharge and plan community care collaboratively with healthcare professionals.
- Collaborate effectively with multi-agency partners including health services, housing providers, the voluntary sector, and families.
- Maintain precise and comprehensive case records, ensuring all statutory documentation is up to date.
- Champion independence and wellbeing while promoting the least restrictive practices.
