What you will do

• Assessment: Undertake assessments of adults’ needs, strengths, and support requirements within the community.
• Care Planning: Develop, implement, and review person-centred support plans that promote independence and wellbeing.
• Case Management: Manage a varied caseload of adults requiring social care support and intervention.
• Multi-Agency Working: Work collaboratively with health professionals, community organisations, carers, and partner agencies.
• Community Visits: Conduct visits across Lewes and North Wealden, including rural locations where access to transport is essential.
• Advocacy: Support individuals to access services and resources that improve their quality of life and achieve positive outcomes.
• Safeguarding: Identify and respond appropriately to safeguarding concerns in line with legislation and best practice.
• Record Keeping: Maintain accurate and timely case records, assessments, and reports.

Qualifications / Requirements

• Social Work Qualification: Degree in Social Work or equivalent recognised qualification.
• Registration: Current Social Work England registration.
• Experience: Previous experience within Adult Social Care and community-based social work teams.
• Assessment Skills: Strong experience undertaking assessments, care planning, and case management.
• Communication Skills: Excellent interpersonal, advocacy, and report-writing abilities.
• Driving Requirement: Access to your own transport is essential due to the rural nature of the service area.
• Partnership Working: Ability to work effectively with multidisciplinary teams and external agencies.
