Locum Social Worker within Adults Hospital Team in Plymouth supporting Cornish clients at Derriford Hospital earning £32.50 hourly. This is an exciting ongoing opportunity to work part-time with a thriving team in a hospital setting, working over 4 days a week with no condensed hours or remote work. The focusing time of 18.5 hours will be spread across 3 days, ensuring you have the perfect life-work balance while making a real difference in people’s lives.

What you will do:

- Conduct timely assessments and ensure patients are on appropriate discharge pathways.
- Collaborate effectively within the Multi-Disciplinary Team (MDT) to provide comprehensive support.
- Implement safeguarding practices for adults, ensuring their safety and well-being.
- Utilise your knowledge of the Discharge to Assess (D2A) process to enhance patient flow.
- Work proactively and efficiently in a busy hospital environment, mainly based at the Derriford office.

Requirements:

- Be present at the Derriford Hospital office for the whole duration of the contracted hours.
- Ability to conduct tasks promptly and efficiently.
- Sound knowledge of safeguarding adults and the Mental Capacity Act.
- Readiness to start as soon as possible and adhere to the regular hours spread over 4 days each week.
