What you will do:
- Conduct comprehensive assessments of adults and carers within diverse settings, collaborating with them to create plans tailored to their specific needs.
- Complete capacity assessments and Best Interest Meetings/decisions as required under the Mental Capacity Act 2005.
- Assist adults who may lack capacity, especially regarding consent to their accommodation and care needs, supporting applications to the Court of Protection for deprivation of liberty authorisation.
- Manage a caseload, delivering social work services to people with care and support needs, including those at risk of harm and neglect.
- Identify and respond appropriately to individuals who may appear to be at risk of significant harm, ensuring their safety while respecting their right to self-determination when they have capacity.
- Collaborate within a multi-disciplinary team to carry out assessments and planning.
- Liaise with other social service professionals, health professionals, and partner agencies.
- Undertake necessary administration, including keeping up-to-date records and managing your workload effectively.

Requirements:
- Registration with Social Work England.
- Degree or equivalent qualification in Social Work.
- Demonstrable UK qualified experience in social work related to adult care and discharge processes.
- Experience with capacity assessments, safeguarding, multi-disciplinary teams, and record management.
