What you will do:
- Participate in duty rota, ensuring that all cases are handled efficiently and effectively.
- Conduct Care Act assessments and develop support plans tailored to the individual needs of our residents.
- Lead Section 42 enquiries to ensure the safety and wellbeing of vulnerable adults.
- Collaborate closely with district nurse colleagues and other stakeholders to provide holistic and integrated care.
- Maintain meticulous records, both manually and electronically, in line with our high-quality service standards.
- Work flexibly across various tasks, demonstrating excellent organisational and multitasking skills.
