What you will do:

• Participate in duty rota, ensuring that all cases are handled efficiently and effectively.
• Conduct Care Act assessments and develop support plans tailored to the individual needs of our residents.
• Lead Section 42 enquiries to ensure the safety and wellbeing of vulnerable adults.
• Collaborate closely with district nurse colleagues and other stakeholders to provide holistic and integrated care.
• Maintain meticulous records, both manually and electronically, in line with our high-quality service standards.
• Work flexibly across various tasks, demonstrating excellent organisational and multitasking skills.
