• Conduct assessments and dynamic risk assessments.
• Perform Mental Capacity Assessments.
• Operate as a Duty rota worker and manage telephone triage.
• Respond swiftly to crisis and safeguarding concerns with appropriate enquiries.
• Collaborate with a multidisciplinary team to deliver integrated health and social care solutions.
• Utilise exceptional interpersonal skills to communicate effectively and be a supportive team player.
