What you will do

Conduct comprehensive Care Act assessments to identify individual needs and develop tailored support plans.

Support carers and promote the wellbeing and independence of adults within the community.

Respond to urgent situations as part of a supportive duty rota, assessing needs and providing immediate assistance where required.

Undertake safeguarding work to protect adults at risk, ensuring their wellbeing and safety.

Apply the principles of the Mental Capacity Act confidently, carrying out risk assessments and positive risk management to help individuals achieve their goals.

Manage a diverse caseload, prioritising effectively to meet the varying needs of the adults you support.
