What you will do:
- Work closely with care leavers to assess their needs and devise effective action plans.
- Manage a variety of cases, ensuring that each young person receives tailored support.
- Facilitate the creation and review of pathway plans that accommodate each individual’s unique journey towards independence.
- Coordinate closely with other professionals and agencies to ensure a seamless support network for the young people in your care.
- Provide comprehensive reports and maintain meticulous records of interactions and developments.
- Attend pivotal meetings and contribute to discussions, ensuring that the voices of the young adults are heard and priorities understood.
- Uphold the local authority’s statutory responsibilities as they pertain to care leavers, ensuring all legal frameworks are adhered to.
- Stay abreast of relevant issues affecting care leavers, ranging from housing to employment opportunities.
- Engage in regular professional development activities to enhance service delivery.
