What you will do:
- Conduct comprehensive assessments to identify the needs of adults within the community, planning and delivering effective interventions.
- Work collaboratively with a range of professionals to design care plans that meet the diverse needs of individuals in the community.
- Provide expert consultation and supervision to colleagues and other professionals within the team.
- Maintain accurate and up-to-date records in accordance with organisational and regulatory guidance.
- Monitor and evaluate care plans and interventions, ensuring high standards of practice are achieved and sustained.

Requirements:
- Registered with Social Work England.
- Degree or equivalent qualification in Social Work.
- Qualified experience in Social Work within the UK is essential for this role to ensure the application of best practices and effective service delivery.
