About the Role:
Westminster City Council is on the lookout for an experienced Mental Health Social Worker who is proactive and innovative, eager to join our renowned South Locality Adult Social Care Mental Health Team. This is your chance to work with a multi-agency team to deliver top-notch social care support to adults in our community. Prepare for an engaging role in which you'll be a linchpin in facilitating residents' access to vital health and community services, driving positive change.
What you will do:
- Participate in the daily duty rota and handle incoming calls and referrals
- Undertake Care Act strengths-based assessments
- Develop and review Care Act care and support plans
- Apply for necessary funding for placements
- Collaborate with health colleagues to craft Section 117 aftercare plans
- Attend MDT ward rounds and community MDT meetings
- Plan safe and timely discharges for individuals in acute or psychiatric hospital settings
- Act as a Lead Enquiry Officer for safeguarding enquiries
- Conduct Carers Assessments under the Care Act
- Undertake Mental Capacity Act assessments
- Contribute positively to team meetings and overall team development

Essential Skills, Qualifications and Experience:
- Registered with Social Work England
- Experience in a community mental health team
- Thorough understanding of the Care Act, Mental Capacity Act, and safeguarding processes
- Ability to work diligently with health professionals and multi-disciplinary teams
- Strong organisational skills, able to manage tasks efficiently in a fast-paced environment
- Compassionate practice with a focus on dignity, respect, and person-centred values
