This is an excellent opportunity for an experienced Senior Social Worker to join the Mental Health Hospital Discharge Team in Ealing. In this role, you will play a key part in supporting safe and effective hospital discharges, ensuring individuals receive the right care and support within the community.

What You Will Do:

• Conduct Care Act assessments and reviews for adults with mental health needs
• Develop personalised, strengths-based care and support plans
• Undertake Mental Capacity assessments and prepare Social Circumstances Tribunal reports
• Act as Social Supervisor for conditionally discharged patients (s37/41)
• Participate in safeguarding enquiries and collaborate with safeguarding teams
• Contribute to duty rota, screening referrals and responding to queries

Requirements:

• Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983
• Experience in hospital discharge and mental health social work
• Familiarity with Court of Protection (Schedule 21A) is advantageous
