What you will do

• Conduct assessments and reviews of social care needs for adults under the Care Act.
• Develop person-centred, strengths-based support plans.
• Undertake capacity assessments under the Mental Capacity Act.
• Prepare Social Circumstances Tribunal reports.
• Take on the role of a Social Supervisor for conditionally discharged s37/41 patients.
• Act as a Safeguarding Enquiry Officer, collaborate with Safeguarding Adults Managers.
• Participate in a duty system for screening referrals and handling safeguarding concerns.
