We are seeking an experienced Senior Practitioner to join the Case Management Team within Adult Services in Hackney. You will play a key role in delivering high-quality case management services while supporting staff development and service improvement.

Key Responsibilities

• Supervise and coordinate tasks to ensure effective delivery of Adult Services objectives.

• Appraise and supervise staff, promoting continuous improvement and high professional standards.

• Ensure compliance with local and national Safeguarding Adults policies and relevant legislation.

• Promote a strengths-based approach, empowering service users and carers to achieve outcome-focused solutions.

• Manage work allocation, oversee staff performance, and provide direct supervision where required.

• Maintain financial awareness, supporting budgetary responsibilities and resource management.

• Uphold departmental standards and ensure accountability for service delivery.

• Work collaboratively within a multi-disciplinary framework to support health and social care integration.

• Support the Team Manager and Consultant Social Worker in balancing service development with operational practice.

• Contribute to Quality Assurance Frameworks in partnership with relevant teams and stakeholders.
