What you will do:

- Carry out specialist functional assessments for adults and young people with visual impairment, identifying needs relating to mobility, independent living skills, communication, and safety.
- Develop and deliver tailored rehabilitation and reablement programmes, including orientation and mobility training, daily living skills, and use of low-vision aids or adaptive equipment.
- Work collaboratively with social care teams, health professionals, education services, and voluntary organisations to ensure coordinated, person-centred support.
- Provide professional advice on sensory impairment, environmental adaptations, and risk management to promote independence and safeguard wellbeing.
- Maintain accurate case records, contribute to support plans, and participate in reviews, ensuring compliance with relevant legislation and council procedures.
- Eligibility requires you to be registered with the Health and Care Professions Council (HCPC) and possess a driving licence to actively engage in a community-based role.
