You will engage in assessing, planning, implementing, and evaluating individual treatment and rehabilitation plans, ensuring a patient-centred approach. What you will do:
- Participate in a multi-disciplinary team to assess, plan, implement, and evaluate individual treatment and rehabilitation plans.
- Use an evidence-based, patient-centred approach to assess, plan, implement, and evaluate treatment plans for residents' physical and psychological needs.
- Work with residents and team members to identify goals and set treatment plans.
- Liaise empathetically with relatives and carers, providing information, support, and advice regarding treatment plans as needed.
- Maintain concise records and provide necessary reports for each resident’s case review.
- Identify risks and advise team members on interventions to minimise these risks.
- Conduct assessments of daily living skills to determine levels of functional ability, risks, and support needs.
Requirements:
- Registration with the Health and Care Professions Council (HCPC).
- A minimum of 2 years of relevant experience.
- Car driver preferred to enable travel between sites and easy access to off-site training opportunities.
