• Play a leading role in planning discharges for individuals returning to their homes.
• Utilise and enhance your skills in moving and handling, focusing on risk assessments and solutions that meet individuals' needs.
• Be a part of the Recovery Hubs, developing a rich, rehabilitation-focused culture across Health and Social Care sectors.
