What you will do
- Reablement Support: Provide specialist occupational therapy support across a 16-bed reablement service.
- Discharge Planning: Facilitate safe and effective discharges from hospital into community settings.
- Assessment: Assess individuals' functional abilities and identify support needs within community environments.
- Care Planning: Develop personalised reablement plans aimed at maximising independence and daily living skills.
- Equipment and Adaptations: Recommend and arrange appropriate equipment and environmental adaptations where required.
- Admission Prevention: Support community step-up admissions and help prevent avoidable hospital admissions.
- Multi-Disciplinary Working: Collaborate closely with healthcare professionals to deliver coordinated care and positive outcomes.
- Professional Practice: Work in accordance with HCPC standards, maintaining high levels of professional and ethical practice.
Qualifications / Requirements
- HCPC Registration: Must be a registered Occupational Therapist.
- Experience: Previous Occupational Therapy experience within community, rehabilitation, or reablement services is desirable.
- Clinical Skills: Strong assessment, rehabilitation, and care planning abilities.
- Communication Skills: Excellent interpersonal and multidisciplinary working skills.
- Problem-Solving: Ability to identify creative solutions to promote independence and wellbeing.
- Professional Knowledge: Strong understanding of reablement principles and person-centred practice.
