What you will do:
- Coordinate care for a complex caseload of service users within the appointed pathway, ensuring comprehensive and personalised care.
- Lead the Care Programme Approach (CPA) process to ensure integrated service delivery and effective care planning.
- Engage in multi-agency working to build collaborative partnerships with other healthcare professionals and agencies.
- Perform risk management tasks to safeguard service users and ensure the provision of high-quality mental health care.
- Administer medications in accordance with professional guidelines and protocols.
- Fulfil the Community Mental Health Team duty worker role on a rolling rota, offering support and expertise when needed.
