What you will do:

- Review existing care packages and support arrangements to ensure optimal outcomes.
- Reassess previous evaluations and compile supporting information for continuity of care.
- Identify and promote opportunities to increase a client's independence and functional capabilities.
- Provide well-rounded recommendations to enhance care and support plans.
- Conduct further Occupational Therapy assessments as required to inform decisions.
- Assess functional abilities and daily living skills to tailor interventions.
- Complete necessary moving and handling assessments for safe and effective practices.
- Foster effective resource utilisation through evidence-based methods.
- Work collaboratively with families, practitioners, and partner organisations for a comprehensive approach.
- Maintain thorough records, reports, and recommendations for continuous service improvement.

Requirements:

- Degree or Diploma in Occupational Therapy and HCPC registration.
- Experience working with children and young individuals with disabilities.
- Proficient in functional assessment and moving and handling assessment experience.
- Knowledgeable in independence-focused practice.
- Capable of managing a varied and dynamic caseload.
- Strong skills in communication, assessment, and report writing.
- Experience within Children's Disability Services and reviewing care packages.
- Contributions to service improvement initiatives and working within multidisciplinary teams.
