Working autonomously while closely collaborating with wider adult social care services, you will play a key role in assessment, intervention, and service coordination, ensuring person-centred and outcome-focused practice.

What You Will Do

  • Carry out comprehensive occupational therapy assessments to identify needs and promote independence within the community.

  • Develop, implement, and review intervention plans that support reablement and functional outcomes.

  • Work collaboratively with the Adult Social Care Team to design effective strategies and interventions.

  • Support individuals through transitions, enabling them to regain confidence and independence at home.

  • Provide specialist advice and guidance to service users, families, and carers.

  • Contribute to multi-disciplinary working to ensure coordinated, person-centred care.

  • Travel across the Horsham area to deliver community-based reablement services using your own vehicle.

  • Maintain accurate and timely records in line with local authority policies and professional standards.

Requirements

  • HCPC registration as an Occupational Therapist.

  • Full UK driving licence and access to a vehicle.

  • Experience within adult social care or community-based occupational therapy is desirable.
