What you will do:

Conduct engaging one-on-one sessions and lead dynamic group work, including SMART groups, to inspire clients in their recovery journey
Manage an active caseload of around 40-45 clients, offering tailored support for those on opioid substitution therapy, with dependency challenges, or low-level substance use
Maintain diligent records through comprehensive compliance documentation, including Treatment Outcome Profiles, care plans, and risk assessments, ensuring excellence in client care
Perform thorough assessments with a deep understanding of substances, harm reduction strategies, relapse prevention, needle exchange protocols, and safeguarding measures
Navigate case management systems with confidence, showcasing your ability to adapt swiftly to our team processes
Work closely with team members to provide holistic support, enhancing service delivery and creating a nurturing environment for clients and colleagues alike
