Dual Care Coordinator / Care Assessor
North Wales | Full Time | £33,000 per annum
Beluga Rox is recruiting on behalf of a multi-award-winning Complex Care provider that delivers specialist care and support services across the UK.
Due to continued growth, we are looking for an experienced and motivated Care Coordinator / Care Assessor to join an established and supportive North Wales team.
This is an excellent opportunity for someone with domiciliary or community care experience who is passionate about delivering high-quality, person-centred care and wants to progress within a respected and growing organisation.
The Role
Working closely with the Registered Manager and wider operations team, you will play a key role in ensuring the smooth coordination and delivery of care services across North Wales.
You will be responsible for coordinating care packages, completing assessments and reviews, supporting care staff, and ensuring clients receive safe, effective, and compassionate care tailored to their individual needs.
This is a varied and rewarding role where no two days are the same.
Key Responsibilities
- Coordinate and schedule care visits efficiently to ensure continuity of care
- Carry out new client assessments and regular care reviews
- Develop and update person-centred care plans
- Maintain accurate records using digital care systems
- Support and communicate effectively with carers, clients, families, and healthcare professionals
- Work closely with the Registered Manager and operations team to ensure excellent service delivery
- Participate in the on-call rota and provide emergency support when required
- Liaise with the complex care nursing team regarding client-specific training and competencies
- Uphold high standards of care and promote the organisation’s reputation as a quality care provider
What We’re Looking For
- NVQ Level 3 in Health & Social Care (or above)
- Previous experience within domiciliary, community, or complex care
- Experience coordinating care within a fast-paced environment
- Strong organisational and communication skills
- A compassionate, proactive, and professional approach
- Confident using IT systems including Word and Excel
- A full UK driving licence and access to your own vehicle
- A genuine passion for delivering outstanding care and support
