What You Will Do:

- Collaborate seamlessly with the Lead Pharmacist and local services to deliver a top-tier clinical pharmacy service across five inpatient wards, the HBPoS, the Hounslow Crisis Team, and the Mental Health Crisis Assessment Service (MHCAS).
- Develop and implement medication management plans tailored to patient needs, ensuring optimal clinical outcomes.
- Engage in patient consultations and provide expert advice on medication use and general health issues.
- Stay abreast of new pharmaceutical approaches and contribute to continuous quality improvements in pharmacy services.
- Support and mentor junior pharmacists and pharmacy technicians, fostering a supportive team environment.
