What you will do

Deliver expert clinical pharmacy services to medical and surgical inpatients.
Undertake medicines reconciliation, clinical screening, and comprehensive medication reviews.
Work collaboratively with doctors, nurses, and allied health professionals to optimise patient care.
Identify, prevent, and resolve medication-related issues to improve safety and outcomes.
Provide pharmaceutical advice and support to ward-based teams.
Contribute to medicines optimisation initiatives and service improvements.
Support and supervise junior pharmacy staff and trainees where appropriate.
Maintain accurate clinical documentation in line with local policies and regulatory standards.
