What you will do:

• Manage a complex caseload of patients with neurological conditions and stroke.
• Undertake specialist assessments and treatment programmes.
• Deliver rehabilitation within patients' homes and local community settings.
• Work collaboratively within a multidisciplinary team.
• Support and supervise junior staff.
• Contribute to patient-centred rehabilitation and recovery plans.
• Travel across Havering and Barking & Dagenham as service needs require.

Requirements:

• Qualified Physiotherapist with HCPC registration.
• Experience in Neurology or Stroke Rehabilitation.
• Community rehabilitation experience.
• Ability to manage a complex caseload independently.
• Strong multidisciplinary working skills.
• Driving licence and access to a vehicle are mandatory.

Desirable skills:

• Previous NHS community experience.
• Experience supervising junior staff.
• Experience working across multiple community sites.
