What you will do

Undertake comprehensive physiotherapy assessments for patients with complex physical needs in community environments, including patients’ homes, clinics, and care settings.

Develop, implement, and review personalised rehabilitation and treatment programmes.

Manage a varied caseload including long-term conditions, frailty, mobility issues, post-acute rehabilitation, and falls prevention.

Provide specialist physiotherapy interventions and advice to patients, carers, and other healthcare professionals.

Support early discharge planning and admission prevention through timely rehabilitation and intervention.

Work autonomously, applying advanced clinical reasoning and decision-making skills.

Supervise and support Band 5 physiotherapists, assistants, and students, promoting high standards of practice.

Contribute to service development, audits, and quality improvement initiatives.

Work collaboratively with MDT colleagues including occupational therapists, nurses, GPs, and social care teams.

Liaise with acute services and external partners to ensure continuity of care.

Maintain accurate, timely clinical documentation in line with NHS Fife policies and professional standards.
