What you will do:
- Conduct comprehensive assessments of patients recovering from strokes, identifying their individual needs and challenges.
- Develop and implement personalised treatment plans aimed at enhancing patients' independence and quality of life.
- Collaborate with a multidisciplinary team to coordinate effective care and ensure seamless service delivery.
- Engage in discharge planning, liaising with patients, families, and community services to ensure smooth transitions.

Requirements:
- Registration with the Health and Care Professions Council (HCPC).
- A minimum of 2 years’ experience working as an Occupational Therapist, preferably in an inpatient rehabilitation setting.
- Experience in stroke management and discharge planning.
- A car driver is preferable for visits and community-based interventions.
