What you will do:

  • Independently hold, prioritise, and manage a diverse caseload with the support of long-arm advice and supervision.
  • Conduct comprehensive assessments and interventions for adults, focusing on complex cases including older individuals with frailty, falls, dementia, and neurological conditions such as Multiple Sclerosis (MS) and Parkinson's Disease (PD).
  • Engage in hospital discharges, rapid response, and preventative measures for admissions.
  • Employ holistic assessment techniques incorporating pressure management, nutrition, and hydration.
  • Drive across different hubs in the area, requiring access to your own car with business insurance.
  • Provide guidance and leadership to your team, handling complex situations and working collaboratively with multidisciplinary teams.
