What you will do:

- Conduct complex health assessments as needed, collaboratively planning and delivering care in partnership with service users.
- Employ the Care Programme Approach (CPA) according to Trust policy, offering specialised care, education, and advice to those with severe and enduring mental health challenges. Act as care co-ordinator when required, including for adults at risk and following supervision procedures.
- Create, implement, and evaluate personalised care plans alongside service users, carers, and professionals, ensuring they reflect individual needs.
- Provide specialist assessment and advice to other agencies and professionals. Participate in multi-agency meetings to offer insights and guide ongoing treatment and care while adhering to National Service Framework (NSF) guidelines.
