What you will do:
- Manage a defined caseload of adults with complex and enduring mental health needs, acting as Care Coordinator where appropriate.
- Undertake comprehensive mental health, risk and needs assessments, formulating and reviewing person-centred care plans.
- Deliver evidence-based therapeutic interventions and recovery-focused treatments within community settings and patients' homes.
- Monitor, assess and manage clinical risk, including safeguarding concerns, ensuring timely escalation where necessary.
- Work collaboratively with psychiatrists, psychologists, occupational therapists, social workers, primary care services, and voluntary sector partners.
Person Specification:
- Hold qualifications as a Registered Mental Health Nurse (RMN) with either a degree or diploma in Mental Health Nursing.
- Demonstrate evidence of continuing professional development relevant to community mental health practice.
- Possess mentorship, supervision, or leadership training, which will be considered desirable.
- Maintain current registration with the Nursing and Midwifery Council (NMC).
- Have significant post-registration experience within adult mental health services and experience working within a Community Mental Health Team, Crisis Service, Early Intervention Service, or similar secondary mental health setting.
