Are you looking for an invigorating locum role where no two days are the same? We have an exciting opportunity for a Band 6 Community Psychiatric Nurse to join the Primary Care Mental Health Team (PCMHT) in Downham. This front-door service supports the Downham, Bellingham, and Catford areas, delivering timely screening, triage, assessment, and short-term evidence-based psychosocial interventions for a diverse range of mental health presentations. You will play a vital role in ensuring responsive, person-centred care while working closely with a multidisciplinary team.

What you will do:

Manage and triage referrals, ensuring timely screening and appropriate allocation.

Complete comprehensive mental health assessments and formulate evidence-based care plans.

Deliver short-term, evidence-based psychosocial interventions tailored to individual needs using dialog.

Undertake risk assessments and contribute to MDT discussions to support effective decision-making.

Work collaboratively with the wider multidisciplinary team to promote recovery-focused outcomes.
Requirements:

Registered Mental Health Nurse (RMN) with valid NMC registration.

Experience working within community mental health services is essential.

Strong assessment, risk management, and communication skills.

Ability to work autonomously while contributing effectively to a multidisciplinary team.
