Job Title: Band 6 Community Psychiatric Nurse - Adult CMHT

Salary: £26 Hourly
Contract: Minimum of 6 months, likely to be ongoing
Hours: Core hours are 9am-5pm Monday - Friday, although flexibility around start & finish times is available. For example, 8am-6pm for 4 days a week, 8am-4pm, 9am-6pm, etc. Part-time considered.

What you will do:
- Deliver community-based psychiatric care to adults, ensuring a high standard of service and support.
- Conduct assessments and create care plans tailored to individual patient needs.
- Work collaboratively within the multidisciplinary Adult Community Mental Health Team to provide holistic care.
- Administer medications and monitor their effects, adjusting care plans as necessary.
- Offer support and advice to patients and their families, promoting mental health education.
- Maintain accurate records and documentation, ensuring compliance with NHS standards and policies.
- Participate in team meetings and contribute to the development of care strategies.
