What You Will Do

• Conduct comprehensive assessments and develop tailored recovery plans for service users.

• Provide ongoing one-to-one support to individuals with long-term mental health and substance misuse issues.

• Work collaboratively with a multi-disciplinary team to improve service user outcomes.

• Maintain accurate, timely, and high-quality records and documentation.

• Promote service user empowerment and support access to appropriate community resources.

Key Experience Required

• Proven experience supporting people with long-term mental health conditions.

• Strong background in substance misuse services.

• Ability to carry out assessments, recovery planning, and provide ongoing support.

• Confidence working independently and as part of a multi-disciplinary team.

• Strong communication and record-keeping skills.
