What You Will Do

Conduct comprehensive assessments and develop tailored recovery plans for service users.

Provide ongoing one-to-one support to individuals with long-term mental health and substance misuse issues.

Work collaboratively with a multi-disciplinary team to improve service user outcomes.

Maintain accurate, timely, and high-quality records and documentation.

Promote service user empowerment and support access to appropriate community resources.

Key Experience Required

Proven experience supporting people with long-term mental health conditions.

Strong background in substance misuse services.

Ability to carry out assessments, recovery planning, and provide ongoing support.
