What you will do:
- Provide specialist assessment, planning, implementation, and evaluation of care for adults with diabetes, including complex cases.
- Manage a defined caseload and support patients to effectively self-manage their condition.
- Deliver evidence-based advice on diabetes treatments, including insulin therapy and newer glucose-lowering agents.
- Offer inpatient diabetes support and facilitate safe discharge planning where applicable.
- Deliver structured education and training to patients, carers, and healthcare professionals.
- Act as a clinical expert and resource within the multidisciplinary team.
- Contribute to service development, clinical audit, quality improvement, and governance activities.
- Support, mentor, and supervise junior staff and students.
- Maintain professional registration and continuing professional development in line with NMC requirements.
