What you will do:

Implement and evaluate physiotherapy treatments aimed at improving patient mobility and function.
Collaborate with a multidisciplinary team to ensure comprehensive care management.
Manage discharge planning effectively to ensure patient safety and continuity of care.
Address specialised needs related to frailty, falls, and cognitive impairments.
Maintain detailed and accurate patient records.
Adapt treatment based on patient progress and evolving clinical guidelines.

Requirements:

Experience in discharge planning
Familiarity with managing frailty and falls
Strong understanding of cognitive impairments
HCPC registered
Driving licence to travel between sites when necessary
