What you will do:
- Deliver person-centred therapeutic support to individuals with learning disabilities and/or autism
- Collaborate with the Multi-Disciplinary Team to design, implement, and review personalised therapy plans
- Offer leadership and mentorship to junior staff, fostering an enabling-focused practice across the service
- Work in partnership with families, carers, and other professionals to ensure a holistic approach to patient care
- Provide flexible support across the Adult Disability Service, focusing on achieving therapeutic outcomes and personal goals
- Assess, develop, implement, and evaluate care plans to maintain high standards of evidence-based care
- Autonomously manage changes in patient presentations, escalating to senior staff when necessary
- Guide and support patients in activities of daily living, promoting independence and quality of life
- Participate in risk management and Quality and Safety processes, ensuring patient safeguards are in place
Requirements:
- NMC Registered, with qualifications as a Registered Nurse LD/RMHN
- Experience or a willingness to train as a mentor, student nurse assessor, or supervisor
- Strong understanding of HSC CARE Values, with skills in planning and prioritising workloads
- Excellent written and verbal communication skills to handle complex situations calmly
- Ability to work independently and as part of a multi-disciplinary team, supporting junior staff
- Punctual, reliable, and equipped with good IT skills; a valid driving license is essential
- Empathetic nature with a good understanding of learning disabilities and/or autism
- Commitment to providing person-centred care and support
