What you will do:
- Undertake holistic nursing assessments and develop person-centred care plans for individuals receiving community nursing services.
- Deliver evidence-based nursing interventions including wound care, medication administration, catheter care, palliative care and long-term condition management.
- Monitor patients' physical health, identify deterioration and escalate concerns appropriately.
- Support hospital discharge pathways and provide rehabilitation-focused interventions to maximise independence.
- Work collaboratively with GPs, Allied Health Professionals, Social Workers, care providers and family members to coordinate care.
Person Specification
Qualifications
- Registered Nurse Adult qualification.
- Current registration with the Nursing and Midwifery Council (NMC).
- Evidence of ongoing Continuing Professional Development (CPD).
- Community nursing qualification or willingness to undertake further development is desirable.
Experience
- Experience working as a Registered General Nurse within NHS, community, primary care, hospital or adult care settings.
- Experience managing patients with complex physical health needs and multiple long-term conditions.
- Experience providing care within patients' homes or community environments is desirable.
