What you will do:

• Assist band 5 and band 6 therapists in creating and managing effective discharge plans for patients.
• Contribute to the writing and updating of patient care plans, ensuring clear and co-ordinated transitions.
• Collect and document patient information to support informed decision-making processes.
• Collaborate with multidisciplinary teams to enhance patient care and streamline discharge processes.
• Provide practical support to patients and their families, offering guidance and reassurance throughout the discharge journey.
