Key Responsibilities / What You Will Do

• Complete comprehensive assessments of older adults to identify needs and strengths.
• Develop personalised care plans that promote independence, wellbeing, and safety.
• Work collaboratively with multidisciplinary teams and external agencies to deliver joined‑up support.
• Provide expert advice, mentoring, and support to less experienced social workers.

Requirements / Candidate Criteria

Essential

• Qualified Social Worker with recognised Social Work degree or equivalent.
• Registered with Social Work England.
• Strong post‑qualification experience in Adults Social Care, specifically with older people.
• Excellent understanding of relevant legislation, including the Care Act 2014.
• Proven ability to manage complex caseloads confidently and independently.
• Strong communication, assessment, and care‑planning skills.

Desirable

• Experience working within a specialist Older People’s or Community Social Work Team.
• Practice teaching, mentoring, or supervisory experience.
• Familiarity with Wakefield’s local pathways and community support services.
