Key Responsibilities / What You Will Do
- Complete comprehensive assessments of older adults to identify needs and strengths.
- Develop personalised care plans that promote independence, wellbeing, and safety.
- Work collaboratively with multidisciplinary teams and external agencies to deliver joined‑up support.
- Provide expert advice, mentoring, and support to less experienced social workers.

- Maintain detailed and accurate case records, ensuring compliance with statutory requirements and best practice.
Requirements / Candidate Criteria
Essential
- Qualified Social Worker with recognised Social Work degree or equivalent.
- Registered with Social Work England.
- Strong post‑qualification experience in Adults Social Care, specifically with older people.
- Excellent understanding of relevant legislation, including the Care Act 2014.
- Proven ability to manage complex caseloads confidently and independently.
- Strong communication, assessment, and care‑planning skills.
Desirable
- Experience working within a specialist Older People’s or Community Social Work Team.
- Practice teaching, mentoring, or supervisory experience.
- Familiarity with Wakefield’s local pathways and community support services.
